IVF Protocols

Pre-IVF Laboratory Tests

  • Antiphospholipid antibody panel (APA)
  • Antithyroid antibody panel (ATA)
  • Immunophenotype (IP)
  • Natural Killer cells with IVIg (NK+IVIg)
  • (DQA1 on partners is recommended)

Treatment

  • APA+
    • Heparin 5000 u sq BID 2 weeks prior to transfer.
    • Aspirin 81 mg QD 2 weeks prior to transfer.

      If patient has PE or PS IgG or IgM, IVIg 500 mg/kg X 1 dose, 5 to 7 days prior to transfer.

    • IVIg is also indicated for patients who have had 4+ failed IVFs and APA+.
  • ATA+
    • Heparin 5000 u sq BID 2 weeks prior to transfer.
    • Aspirin 81 mg QD 2 weeks prior to transfer.
    • IVIg 500 mg/kg X 1 dose, 5 to 7 days prior to transfer.

  • IP - Abnormal CD 56+/CD 16+ or CD 5+/CD 19+
    • IVIg 400 mg/kg X 3 consecutive days.
    • Repeat IP q month and treat with IVIg until IP normalizes.

  • NK+IVIg - same as for abnormal IP.
Heparin and Aspirin are continued through week 12. If patient also has a history of recurrent miscarriage, Heparin and Aspirin should be continued through week 34.
