IVF Protocols
Pre-IVF Laboratory Tests
- Antiphospholipid antibody panel (APA)
- Antithyroid antibody panel (ATA)
- Immunophenotype (IP)
- Natural Killer cells with IVIg (NK+IVIg)
- (DQA1 on partners is recommended)

Treatment
- APA+
- Heparin 5000 u sq BID 2 weeks prior to transfer.
- Aspirin 81 mg QD 2 weeks prior to transfer.
If patient has PE or PS IgG or IgM, IVIg 500 mg/kg X 1 dose, 5 to 7 days prior to transfer.
- IVIg is also indicated for patients who have had 4+ failed IVFs and APA+.
- ATA+
- Heparin 5000 u sq BID 2 weeks prior to transfer.
- Aspirin 81 mg QD 2 weeks prior to transfer.
- IVIg 500 mg/kg X 1 dose, 5 to 7 days prior to transfer.
- IP - Abnormal CD 56+/CD 16+ or CD 5+/CD 19+
- IVIg 400 mg/kg X 3 consecutive days.
- Repeat IP q month and treat with IVIg until IP normalizes.
- NK+IVIg - same as for abnormal IP.
Heparin and Aspirin are continued through week 12. If patient also has
a history of recurrent miscarriage, Heparin and Aspirin should be continued
through week 34.
- Laboratory tests are repeated every trimester.
